Increase in healthcare salaries, the ok arrives for doctors and other managers with the new Ccnl

After months of waiting and negotiations, the Council of Ministers has decided to authorize the Minister for Public Administration, Paolo Zangrillo, to express a favorable opinion on the Ccnl which directly concerns the management staff of the Healthcare area. It is not just an increase in the pay slip, but a broader intervention that affects wages, allowances, work organization, protection and career.

The innovations introduced concern in particular doctors, veterinarians, healthcare managers and public health professionals.

Ccnl Sanità, government’s green light for increases

The hypothesis of a national collective bargaining agreement for management personnel in the Healthcare area was signed on 18 November 2025 by Aran and the trade union organisations.

The favorable opinion of the Regions-Health Sector Committee also arrived in December. With the approval of the Council of Ministers, which arrived on 5 February 2026, the contract now enters the final phase, allowing the concrete application of the increases and new rules envisaged.

The contract applies to all medical, healthcare, veterinary and health professions managers, as defined by thearticle 7, paragraph 5, of the Ccnq dated 22 February 2024.

Both permanent and fixed-term workers are included, as long as they are employees of companies and institutions in the healthcare sector. Also included are often less visible but strategic bodies, such as the experimental zooprophylactic institutes (Izs) and the regional agencies for environmental protection (ARPA).

In total, the audience of recipients is over 137 thousand personnel, according to data updated as of 31 December 2021.

Salary increases: how much the paycheck grows

The heart of the contract is represented by salary increases. Once fully operational, starting from 1 January 2024, the Ccnl recognizes increases equal to 5.76% of the total wages useful for contractual purposes.

A percentage which, although not entirely compensating for the loss of purchasing power accumulated in the years of high inflation, nevertheless represents a significant step forward.

In concrete terms, the increase in the standard salary is on average equal to 230 euros per month for 13 months. It is therefore approximately 3,000 euros gross per year, which is added to other increases linked to additional salary items.

What changes for healthcare managers

In addition to the basic salary, the contract affects various fundamental components of the remuneration of healthcare managers, a category which includes, for example, all doctors and veterinarians employed by healthcare companies.

For this specific category, increases in the values ​​of position pay, medical-veterinary and healthcare specific allowance and allowance for complex management assignment are envisaged.

The re-establishment of the employment relationship is also envisaged, a measure designed to facilitate the return of managers who have interrupted their service.

The introduction of the judicial police allowance for managers responsible for official controls and other inspection activities is expected. This is recognized to those who effectively carry out control functions and have the qualification of judicial police agent or officer.

Until now, the use of additional services was often poorly planned, spending could grow unevenly across healthcare companies and, in some cases, excessive and continuous use was created, with negative effects on costs, organization and workloads. The contract therefore introduces an annual spending ceiling, which each healthcare company will have to manage and not exceed.

News on career and probationary period

The Ccnl then rewrites the rules on the probationary period, defining them in a more precise way duration, methods and purposes for each figure and path (overcoming some ambiguities of previous contracts).

Furthermore, this is considered as experience for the purpose of assigning roles, thus recognizing skills and seniority often excluded from formal counts.

Finally, a particularly current chapter concerns safety at work. The Ccnl provides legal aid in the event of attacks suffered by management personnel by third parties.
